No-Pet and No-Animal Affidavit
When a household will have no household pets and no assistance animals, each applicant age 18 or older must individually complete and submit their own No-Pet and No-Animal Affidavit.

Confirm Which Pet or Animal Documentation Applies
The required documentation depends on whether the household will have no pets or assistance animals, will have a household pet, or includes an applicant requesting a disability-related reasonable accommodation for an assistance animal.
If the household’s circumstances change: Complete the applicable documentation before bringing or allowing a household pet at the rental property. A disability-related request for an assistance animal must be submitted through the separate reasonable accommodation process.

One applicant cannot complete this affidavit for another applicant or submit one affidavit on behalf of the entire household.
