I affirm that I do not currently own a pet or animal (of any size/kind and kept inside/outside) in the rental unit that I currently lease or intend to lease from my property manager/landlord.

I understand and agree that prior written permissions from my landlord/property manager is required to own a pet or animal of any size and kind, inside or outside, during my tenancy. If permission is granted by my landlord/property manager then it also requires completing a new affidavit for the pet or animal.

I understand and agree that absolutely NO pet or animal sitting or visitation (for any length of time ad inside or outside my rental unit) is allowed during my tenancy unless I have prior written permission from my landlord/property manager.

I understand and agree that I may be subject to eviction proceedings and monetary fines if an unauthorized pet/animal is identified in my rental unit during my tenancy.

I understand and agree that my landlord/property manager may have additional no pet/animal policies than what is stated herein this application. It is my responsibility to read, review and abide by their policies at all times during my tenancy.

I understand this no-cost ($0.00) affirmatino process is a requirement for anyone in the household that either has no pet or is also co-pet owner where at lease one person in the household has completed the Pet profile affidavit process
