Complete the Household Pet Affidavit

Submit one Household Pet Affidavit for each household pet. Only one household-wide affidavit and profile is needed for each pet, regardless of how many applicants are in the household. Do not submit duplicate affidavits for the same pet.

Provide complete and accurate information about the pet below. This affidavit helps Rice Real Estate & Property Management review the pet for the specific rental property and document the household’s pet information.

No cost: There is no cost to complete the household-pet affidavit.
One form per pet: Submit a separate affidavit for each household pet expected to live at the property.

Pet approval is separate: Completing this affidavit does not approve the pet. Household-pet approval is property-specific and requires written approval from Rice Real Estate & Property Management before the pet may live at the rental property.

Service animals and other disability-related assistance animals are not reviewed as household pets. Those requests must use the separate reasonable accommodation request process .
