The required documentation depends on whether the household has no pets or assistance animals, one or more household pets, one or more assistance animals, or a combination of both.
Who Must Complete Each Form?
First identify which situation applies. Then follow the individual or household-wide submission rule shown on that option.
Individual Form Required
Your Household Has No Pets or Assistance Animals
Each proposed occupant age 18 or older must complete and submit their own No-Pet and No-Animal Affidavit.
One separate affidavit is required from every proposed adult occupant. One adult cannot complete the affidavit for another adult.
Household-Wide Form
Your Household Has One or More Pets
Submit one Household Pet Affidavit for each pet. Each affidavit applies to the entire applicant household.
Only one affidavit is needed for each pet, regardless of how many applicants are in the household. Do not submit duplicate affidavits for the same pet.
Reasonable Accommodation
Your Household Has One or More Assistance Animals
Submit one Reasonable Accommodation Request for each assistance animal .
Only one request is needed for each assistance animal, regardless of how many applicants are in the household. Do not submit duplicate requests for the same assistance animal.
No-pet household One individual affidavit from each proposed occupant age 18 or older.
Pet or assistance-animal household One household-wide submission for each pet or assistance animal.
