The required documentation depends on whether the household has no pets or assistance animals, one or more household pets, one or more assistance animals, or a combination of both.

Who Must Complete Each Form?

First identify which situation applies. Then follow the individual or household-wide submission rule shown on that option.

Individual Form Required

Your Household Has No Pets or Assistance Animals

Each proposed occupant age 18 or older must complete and submit their own No-Pet and No-Animal Affidavit.

One separate affidavit is required from every proposed adult occupant. One adult cannot complete the affidavit for another adult.

Submit one Household Pet Affidavit for each pet. Each affidavit applies to the entire applicant household.

Important Pet and Animal Documentation Information

Review these requirements before completing the pet or assistance-animal documentation that applies to your household.

Applicant approval does not approve a household pet. Household pets are reviewed separately and require written approval for the specific rental property.
Documented bite history must be disclosed. Rice Real Estate & Property Management reviews the circumstances and available documentation. A documented bite history is not an automatic denial.
Each adult completes their own no-pet affidavit. When the household has no pets or assistance animals, each proposed occupant age 18 or older must individually complete and submit a No-Pet and No-Animal Affidavit. One adult cannot complete the affidavit for another adult.
Submit one household-wide form for each pet or assistance animal. Complete one Household Pet Affidavit for each household pet and one Reasonable Accommodation Request for each assistance animal. Do not submit duplicate forms for the same pet or animal.
Assistance animals are not household pets. A disability-related assistance-animal request is reviewed through the reasonable accommodation process.
